I started with a piece of Balmy Blue Cardstock sized:
9 1/2 x 8 1/2"
I scored the 9 1/2" side at:
1, 4 1/4, 5 1/4 and 8 1/2"
I scored the 8 1/2" side at:
1, 2, 6 1/2 and 7 1/2"

Below is a picture of how your Cardstock should look after scoring and cutting.
I attached several pieces of Designer Series Paper to the box.
Subtles DSP Balmy Blue
(1) 4 1/4 x 1 3/4" front/top
(1) 4 1/4 x 3/4" top long side
(2) 3/4 x 3" short sides
Regals DSP Old Olive
(1) 4 1/4 x 1 1/2" front/bottom
(1) 4 1/4 x 3/4" bottom long side
I stamped images onto Basic White Cardstock.
Tractor and Trailer - Real Red Ink
1 Large and 3 Small Wheels - Smokey Slate Ink
Haystack - Daffodil Delight Ink
Chicken - Pumpkin Pie Ink
I cut the images out using the matching dies.
I attached the Wheels and Haystack to the Tractor and Trailer with Liquid Glue.
I attached the Trailer to the upper/left of the box front with Liquid Glue.
I attached the Tractor with Dimensionals.
I attached the Chicken to the Tractor with a Dimensional.
I stamped the sentiment "HAY" three times onto Basic White Cardstock with Daffodil Delight Ink.
I cut the sentiment out using a die from the Textured Notes dies.
I attached it to the upper right/front of the box, with some hanging off the right side, with Liquid Glue.
I cut the excess off the right side.
For the final touch I added 3 Peach Pie In Color Flat Pearls to the Tractor and Haystack.
